Do not pay the high cost of healthcare anymore, our monthly fee through the medical assurance plan is $65 a month for an individual or $150 per month for a family of 3 or more, which is paid on enrollment and is recurring on the first of each month thereafter. Our Medical Assurance Plan members will receive a yearly exam, and if needed, up to two office visits free of charge each month for healthcare. They will also receive unlimited phone calls for medical advice or medication refill needs. Primary Care of Orange City will maintain the records for, and help the member to obtain, all recommended health screenings and provide prescriptions.
